Cayaba Care’s mission is to improve birth experiences and outcomes in communities impacted by racial and social inequities by reimagining how maternity care is delivered. Our team is focused on building technology-enabled solutions to deliver clinical and non-clinical services in the home and virtually. Home- based maternity support is provided by our team of Maternity Navigators, who are from the communities they serve and trained in maternal community health work including prenatal education, connection to needed resources and advocacy for enrolled members. They work with a multi-disciplinary team of professionals including clinicians, therapists, social workers and lactation consultants to help pregnant and postpartum families. The Cayaba care model focuses on integration, not replacement of members’ primary obstetric provider and reducing barriers to care.

The Role

Cayaba Care is seeking a full-time hourly Maternity Navigator (MN) who will support Cayaba members through the prenatal and postpartum period. The MNs develop supportive relationships with our members, meeting with them monthly to understand their needs and provide key resources throughout their pregnancy journey. This role will report to the Director of Care Operations.

Our team provides care to our members virtually and in person, depending on the member's need and preference.

Work Schedule
This role is expected to work the following schedule:

Monday, Wednesday, Friday: 8:30am-5pm (hybrid)

Tuesday, Thursday: 10am-6pm (hybrid)

Saturday: 1x a month 9am-2pm (remote)

Responsibilities:

  • Actively manage a panel of members through the prenatal and postpartum period, engaging them monthly to offer different resources and levels of support based on their pregnancy phase and needs
  • Conduct in-home and virtual visits, educating members on health and self-advocacy, providing emotional support, monitoring vital signs, and screening for additional needs
  • Identify and connect members to appropriate community resources
  • Proactively identify and contact your assigned members that have not had recent support from Cayaba, by phone or text, to check-in on their needs and offer other resources
  • Screen members for Social Determinants of Health (SDoH) and identify and connect them to appropriate community resources as needed
  • Be available to members via phone to provide support for any concerns between visits
  • Coordinate with the multidisciplinary care team, referring members to Cayaba specialists and sharing member needs during daily care team huddles
  • Document care plans, services provided, referrals, and screening results in the member’s record
  • Escalate any clinical complaints from members to the clinicians
  • Provide active labor support to members as needed
  • Follow our Cayaba policies and procedures to best support our members

Qualifications:

  • Minimum of 1-3 years of experience working with patients in the Women’s health and obstetrics ambulatory care setting
  • Certified Medical Assistant required, Doula certification preferred
  • Strong organizational and problem solving skills, with attention to detail
  • Excellent communication and interpersonal skills
  • Strong desire to build supportive relationships and provide compassionate, culturally sensitive care for our program members
  • Excitement around working with a collaborative interdisciplinary team of specialists
  • Comfort working across multiple technology platforms and learning new technologies

Nice to haves:

  • 1-3 years of experience in community work, patient advocacy, or health navigation
  • Community Health Worker or Breastfeeding Counselor certification
  • Specialized in OB-GYN, Women’s Health
  • Fluent in Spanish or French
  • Start-up experience
