The Role:

We are seeking a compassionate and experienced Registered Nurse (RN) to serve as a vital link between clinical care and community-based support for new mothers and their babies. In this role, you will act as a trusted clinical partner to the families we serve, providing guidance throughout the postpartum period. Responsibilities include assessing the medical needs of mothers and infants following NICU discharge, addressing postpartum concerns, and working closely with Cayaba Maternity Navigators to connect families to non-clinical resources and support.

This role will help shape how we deliver care to families in Washington, D.C., ensuring our model is responsive to their unique needs. This position reports to the Director of Care Operations.

Work Hours:
Part-time position; minimum of three (3) days per week, working between 9:00 AM – 5:00 PM.

Responsibilities:

Clinical Monitoring & Escalations

  • Conduct in-home assessments within 48 hours of an infant’s NICU discharge

  • Lead telemedicine visits at key intervals during the postpartum period to evaluate clinical needs and provide education

  • Assess reports of urgent maternal warning signs or elevated blood pressure and guide members to appropriate care settings

  • Monitor blood pressure and EPDS scores; follow up on any values requiring clinical attention

  • Serve as a liaison between Cayaba Care and external clinical providers, documenting visit summaries and ensuring timely escalation of clinical concerns

Care Quality & Training Support

  • Train Maternity Navigators (MNs) on clinical topics to ensure consistent, evidence-based education across the care team

  • Provide insights from your frontline experience to help shape and improve Cayaba Care’s programs

  • Participate in care team huddles, fostering collaboration and knowledge sharing between clinical staff and MNs

Qualifications:
  • Active Registered Nurse (RN) license in Washington, D.C. and/or a compact state license (BSN or MSN preferred)

  • Based in the Washington, D.C. area with the ability to conduct in-person visits in Southwest and Southeast D.C.

  • Minimum of 5 years of experience in pregnancy and postpartum care in outpatient pediatrics, OB/GYN, maternal-fetal medicine, acute care, or a similar setting

  • Exceptional interpersonal and communication skills with the ability to provide compassionate, empathetic, and culturally-informed care

  • Highly organized and solutions-oriented, with the ability to adapt in a fast-paced, evolving environment

  • Comfortable establishing strong relationships with patients virtually

  • Tech-savvy and comfortable (or willing to learn) AthenaOne, Welkin, and MacOS for care documentation and communication

Preferred: Proficiency in Spanish or Amharic to better serve our diverse member population
