Role Overview

As a NY Case Manager at Daivergent, you will play a central role in ensuring high-quality service delivery across the State of New York for our vocational rehabilitation (VR) and work readiness programs.

This role combines elements of service coordination, coach support, issue resolution, and direct client support. The position is well-suited for mission-driven professionals who value autonomy in managing service delivery responsibilities and collaborating with external partners to support positive client outcomes.

You will support both new and experienced coaches through onboarding support, reinforcing service delivery expectations, and providing real-time guidance related to active services. The role also involves communicating with state disability agencies and school partners to coordinate approved service plans and share client progress. Comfort interacting with clients, service providers, and vocational rehabilitation counselors or administrators is required.

The ideal candidate is detail-oriented, strengths-based, and passionate about improving outcomes for adolescents and adults with disabilities in a remote environment. A portion of your time will be dedicated to visiting schools, community organizations, and state disability offices across New York State.

Key Responsibilities

Client & Agency Coordination

  • Meet in-person with service coordinators, VR counselors, school administrators, and state disability agency staff on a weekly basis at their New York offices.
  • Conduct virtual meetings via Zoom, email, and phone with service coordinators, VR counselors, school administrators, and staff from state disability agencies to provide updates on client progress, address service-related questions, and ensure ongoing alignment.
  • Represent Daivergent with professionalism while communicating client updates, resolving issues, and aligning on service plans.
  • Manage the full client lifecycle from referral through onboarding, contract authorizations, engagement monitoring, and service closure.
  • Monitor paused, stalled, or disengaged cases and complete follow-up or case closure

Coach Training & Support

  • Support the onboarding of new coaches by providing program orientation and practical guidance on delivering services in both individual and group settings
  • Provide state-specific compliance guidance to coaches regarding ACCES-VR service codes, documentation requirements, and data standards for NY.
  • Provide ongoing support to assigned coaches by responding to service delivery questions, assisting with issue resolution and escalations, and reinforcing service delivery expectations.

Documentation & Internal Collaboration

  • Submit detailed, accurate, and timely case notes for all client, school, and agency interactions using Daivergent’s internal systems.
  • Work closely with Daivergent billing team to request authorizations and amendments for client service plans.
  • Conduct regular audits and quality assurance checks of coach notes.
  • Participate in regular case meetings with other Case Managers to review escalations, support complex cases, and ensure consistent service delivery practices across teams.

Key Requirements

Professional Experience

  • 2+ years of direct case management experience in a disability or special education context. Bonus for experience liaising directly with state vocational rehabilitation counselors.
  • 3+ years of direct disability service experience, such as special education, job coaching, behavioral therapy, life-skills training, or related fields.

Education

  • High School/GED required.
  • Bachelor’s degree in disability services, special education, counseling, psychology, social work, or a related field preferred.

Technical & Work Environment

  • Based within 15 miles of New York City: given the importance of our in-person school, family, and VR relationships, the ideal candidate is located near NYC
  • 30% of your time will be spent in-person at our schools, VR agencies, and community organization partners across New York State
  • The remaining time can be work-from-home or in our office in Manhattan
  • Comfortable working in a remote, technology-driven environment.
  • Strong ability to document interactions clearly and maintain organized workflows within digital systems.
