About the Opportunity

Pair Team is hiring a full-time Care Operations Manager to lead and support a team of Community Health Workers (CHWs) and Nurses delivering high-touch, relationship-based care to Medicaid members with complex medical, behavioral, and social needs.

This role plays a key part in ensuring quality and consistency across care delivery, driving member engagement, and coaching CHWs/RNs to succeed in a remote-first, field-integrated care model. The Care Operations Manager will provide day-to-day guidance, performance support, and strategic direction while helping optimize outreach, documentation, and field coordination efforts.

This position primarily allows for remote work; however, it includes regular in-person visits in the community across Travis County (Austin, TX). Typically 1 or 2 days per week will include visiting patients in the community, at their homes and in various sites of care.

What You’ll Do

  • Directly manage and support a team of CHWs & RNs, providing regular 1:1 coaching and performance feedback
  • Ensure high-quality care delivery and documentation across the team through ongoing review and coaching
  • Monitor performance metrics (e.g., outreach, engagement, documentation) and intervene early to address gaps
  • Serve as the first point of contact for escalations related to members, workflow, or field activity
  • Support onboarding and training for new CHWs and contribute to SOP development and updates
  • Foster a collaborative and mission-driven team culture that centers trust-building and member advocacy
  • Collaborate with interdisciplinary teammates (e.g., RN Care Managers, Behavioral Health Care Managers) to ensure coordinated support
  • Participate in team huddles, case reviews, and cross-functional meetings to improve care delivery and communication
  • Participate in field-based member visits as needed to support team development and engagemen

What You’ll Need

  • Active CHW Certification in the state of Texas (preferred)
  • Based in Travis County (Austin, TX) (required)
  • Bilingual in English and Spanish (preferred)
  • 2+ years of experience managing frontline or community-based teams, ideally in healthcare, public health, or social services
  • 2+ years of experience as a community health worker or in a similar outreach or care coordination role
  • Reliable access to a vehicle
  • Reliable internet connection and access to a quiet, HIPAA-compliant workspace
  • Strong communication skills and cultural competency when working with vulnerable populations
  • Ability to work independently in a remote environment while staying aligned with a cross-functional care team
  • Organized, detail-oriented, and comfortable using digital tools (e.g., EHRs, CRMs, video conferencing platforms)
  • Valid driver’s license and active auto insurance
